COMMUNITY LONG-TERM CARE (CLTC)

SERVING SC

Community Long Term Care (CLTC) assists individuals who wish to live at home with additional assistance, provided they meet Medicaid financial eligibility criteria. CLTC services cater to adults aged 18 and older who have difficulty with daily activities, such as bathing, dressing, and using the restroom, due to illness or disability.

COMMUNITY CHOICES

To qualify for the Community Choices waiver, individuals must demonstrate the same level of care required to enter a nursing facility. Services include:

  • Adult Day Health Care(ADHC)

    • Licensed day care center offering medically supervised care and services. Transportation is provided to and from the home within a 15-mile radius of the center.

  • ADHC Nursing

    • Skilled nursing procedures that are limited and ordered by a physician may be provided by the nurse at the ADHC center.

  • Attendant Care

    • Providing direct support with personal care and daily activities to ensure health and safety.

  • Companion Services

    • Services that provide essential short-term relief for caregivers while delivering necessary supervision for clients.

  • Environmental Modification

    • Minor modifications to the home, which include pest control.

  • Home Delivered Meals

    • Regular and specialized dietary meals are available for delivery to clients' homes.

  • Limited Incontinence Supplies

    • Offering a limited supply of absorbent underwear and underpads.

  • Personal Care Service

    • Assists individuals with essential daily activities, including bathing, dressing, meal preparation, housekeeping, and monitoring health indicators.

  • Respite Care

    • Temporary relief for the client's caregiver can be achieved by admission to an inpatient facility, such as a nursing home, hospital, or community residential care facility (CRCF).

  • Personal Emergency Response System (PERS)

    • An electronic device that enables individuals, especially those with disabilities or living alone, to call for help during an emergency.

HIV AIDS

The CLTC program is available to support individuals of all ages living with HIV/AIDS who are at risk of hospitalization. This program offers a range of services under the HIV/AIDS waiver, aimed at addressing the specific needs of the individual.

  • Attendant Care

    • Providing direct support with personal care and daily activities to ensure health and safety.

  • Companion Services

    • Services that offer short-term support for caregivers while ensuring clients receive necessary supervision.

  • Individual Care Service

    • Provides assistance with essential activities, including bathing, dressing, meal preparation, housekeeping, and the monitoring of health indicators.

  • Home Modification

    • Minor improvements to the residence will be implemented, including pest control measures.

  • Meal Delivery

    • We offer regular and specialized dietary meals, delivered directly to your home.

  • Nutritional Supplements

    • This service offers a variety of nutritional supplements.

  • Private Nurse

    • Expert medical monitoring and direct care to effectively address healthcare needs through comprehensive home support.

INTERMEDIATE CARE FACILITIES FOR INDIVIDUALS WITH INTELLECTUAL DISABILITY (ICF/ID)

Individualized care to improve functional abilities and promote independence. This benefit is provided by all states, often as an alternative to home and community-based services for those needing care at the ICF/ID level.

  • Adult Companion Services

    • Non-medical care, supervision, and social activities provided for adults aged 21 and older.

  • Adult Day Health Care

    • Care provided in an outpatient setting that encompasses both health and social services (age 21 or older).

  • Adult Day Health Care Nursing

    • Services provided in the ADHC center include ostomy care, urinary catheter care, wound care, tracheotomy care, tube feedings, and nebulizer treatment for individuals aged 21 and older.

  • Adult Day Health Care Transportation

    • Prior-authorized for participants receiving Adult Day Health Care (ADHC) who reside within 15 miles of the ADHC center.

  • Audiology Services

    • Extension of the State Plan Medicaid benefit for adults 21 years or older.

  • Adult Dental Services

    • Extension of the State Plan Medicaid benefit for adults 21 years or older.

  • Adult Vision

    • Extension of the State Plan Medicaid benefit for adults 21 years or older.

  • Behavior Support

    • Services are available to support participants in acquiring the skills necessary for effective self-management of their behavior.

  • Career Preparation Services

    • Services aim to prepare participants for careers by exposing them to various professions and teaching essential skills such as attendance, task completion, problem-solving, safety, self-determination, and self-advocacy.

  • Employment Services

    • Ongoing support is provided for individuals who are unlikely to achieve competitive employment at or above minimum wage without assistance. Due to their disabilities, these individuals require support to perform effectively in a typical work environment.

  • Environmental Modification

    • Physical adaptations made to the participant’s home to ensure health, safety (e.g., installation of ramps, widening of doorways, etc.).

  • Nursing Services

    • Nursing services ordered by a physician and provided by a licensed nurse.

  • Personal Care Service

    • Support with personal care and daily living activities (e.g., bathing, dressing, meal preparation, laundry, etc.).

  • Personal Emergency Response System(PERS)

    • An electronic device to assist participants at high risk of institutionalization in obtaining emergency help. It is intended for individuals who live alone or spend a significant amount of time alone and would otherwise require extensive supervision.

  • Prevocational Habilitation

    • Preparing an individual for either paid or unpaid employment, without focusing on specific job tasks.

  • Private Vehicle Modification

    • Modifications to a privately owned vehicle used to transport participants.

  • Psychological Services

    • Mental health services are tailored to meet the needs of individuals with disabilities.

  • Respite Care

    • Temporary care is provided due to the absence of regular caregivers.

  • Residential Habilitation

    • Care, adaptive skills training, and supervision provided in a non-institutional setting.

  • Specialized Medical Equipment, Supplies and Assistive Technology

    • Equipment and supplies that are not covered under State Plan Medicaid that provide medical or remedial benefits to the participant. This includes absorbent underwear and underpads for individuals aged three (3) years and older.

  • Support Center

    • Non-medical care support, supervision, and assistance provided in a non-institutional group setting for individuals with disabilities who cannot care for or supervise themselves.

HEAD AND SPINAL CORD INJURY

This program offers services to individuals with traumatic brain injuries, spinal cord injuries, or similar disabilities that are not associated with progressive illnesses or age-related neurological disorders. The goal is to prevent or delay institutionalization in nursing homes or ICF/IIDs. Participants must meet the criteria for Nursing Facility or ICF/IID level of care.

  • Attendant Care

    • Providing direct support with personal care and daily activities to ensure health and safety.

  • Behavior Support

    • Identifying specific challenging behaviors and implementing suitable interventions to mitigate or eliminate these behaviors.

  • Career Preparation Services

    • Services aim to prepare participants for careers by exposing them to various professions and teaching essential skills such as attendance, task completion, problem-solving, safety, self-determination, and self-advocacy.

  • Companion Services

    • Services that offer short-term support for caregivers while ensuring clients receive necessary supervision.

  • Day Activity

    • Support individuals in developing and enhancing self-help, socialization, and adaptive skills within a licensed day program funded by SCDDSN.

  • Employment Services

    • Ongoing support is provided for individuals who are unlikely to achieve competitive employment at or above minimum wage without assistance. Due to their disabilities, these individuals require support to perform effectively in a typical work environment.

  • Environmental Modification

    • Minor modifications to the home, which include pest control.

  • Health Education for Participant-Directed Care

    • Guidance by a registered nurse to help an individual manage their personal care, supported by another person.

  • Independent Living Skills

    • Services aimed at developing, maintaining, and enhancing the community living skills of a waiver participant, including direct training from qualified staff.

  • Individual Care Service

    • Provides assistance with essential activities, including bathing, dressing, meal preparation, housekeeping, and the monitoring of health indicators

  • Medicaid Waiver Nursing

    • Services within the scope of the SC Nurse Practice Act provided by an RN or LPN.

  • Occupational Therapy

    • Treatment to restore or improve fine motor functioning.

  • Personal Emergency Response System (PERS)

    • An electronic device that enables individuals, especially those with disabilities or living alone, to call for help during an emergency.

  • Private Vehicle Modification

    • Modifications to a privately owned vehicle used to transport participants.

  • Psychological Services

    • Treatment to address affective, cognitive, and substance abuse issues.

  • Respite Care

    • Temporary care provided due to the absence of regular caregivers.

  • Specialized Medical Equipment, Supplies, and Assistive Technology

    • Equipment and supplies that are not covered under the State Plan Medicaid that provide medical or remedial benefits to the participant. This includes absorbent underwear and underpads for individuals aged three (3) years and older.

  • Speech and Hearing Services

    • Speech therapy, audiology services, and augmentative communication.
